Job summary

CenterWell Home Health in Texas is seeking an Occupational Therapist to provide in-home therapy, assess daily living skills and develop retraining programs to improve strength and independence.

You will work with physicians and the care team, implement therapy per physician orders, and document progress and outcomes. A valid OT license, CPR certification, and a valid driver’s license with insurance are required.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function in home settings to assist physician evaluation.
• Confer with physicians and health care team to develop/revise the Plan of Care.
• Provide therapies and instruction to improve/restore strength, coordination and function.
• Document patient status, progress and discharge summaries accurately.
