Job summary

CenterWell Home Health is seeking an experienced Occupational Therapist to deliver in-home rehabilitation services. You will assess daily living activities, design retraining plans, and collaborate with physicians and the care team to optimize patient outcomes in a home-based setting.

You will guide patients, families, and staff through therapy goals, safety, and activities of daily living while documenting progress and coordinating with the clinical team to ensure continuity of care.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid drivers license, auto insurance and reliable transportation.

Responsibilities

• Assess daily living and work-related skills; develop retraining programs with measurable objectives.
• Confer with physician and care team to develop/update the Plan of Care.
• Provide therapy to improve/restore strength, coordination, ROM and function per physician orders.
• Review retraining programs to align with patient goals and rehab program guidelines.
• Consult with vocational team to design plans matching patient needs and capabilities.
• Document patient status, progress, and interventions accurately in the patient record.
• Participate in care coordination and discharge planning; support other staff as needed.
• Design community reintegration activities and coordinate with interdisciplinary team.
