Job summary

CenterWell Home Health is seeking an Occupational Therapist to deliver in-home rehabilitation and functional improvement, reporting to the Home Health Branch Administration. You will assess, plan and implement therapy to help patients regain strength and independence in their own homes.

The role requires an OT license, CPR certification, and a valid driver's license with reliable transportation. Prior home health experience is preferred.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient's level of function.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
• Review/expand retraining programs in line with patient's behavioral goals and rehabilitation program guidelines.
• Consult with vocational team members to develop vocationally oriented plans matching patient needs.
• Report patient status and progress to physician, rehabilitation staff, and family.
• Participate in care coordination and discharge planning activities; assist in meeting patient needs.
• Design community reintegration activities to assist patient in physical reconditioning and psychological adjustment.
• Recommend/design adaptive equipment to improve home/work environments and participation in rehabilitation.
• Design/train staff and family members to carry out retraining programs including dressing, feeding, grooming and hygiene skills.
