Job summary

CenterWell Home Health is seeking a dedicated Occupational Therapist to provide home-based therapy, focusing on rehabilitation and functional improvement for patients in their homes. You will assess daily living skills, collaborate with physicians, and deliver targeted therapeutic interventions to restore strength and mobility.

The role requires current OT licensure, CPR certification, and reliable transportation. Prior home health experience is preferred.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patients' daily living and work-related skills and develop retraining programs with measurable objectives.
• Confer with the physician and care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Review and expand retraining programs to align with rehabilitation goals and program guidelines.
• Document patient status, progress and outcomes for the physician and care management staff.
