Job summary

CenterWell Home Health in Florida seeks an Occupational Therapist to help patients regain strength and independence at home. You will assess daily living skills, design therapeutic retraining programs with measurable goals, and work with physicians and the care team to optimize recovery.

This patient‑facing role requires current OT licensure, CPR certification and a valid driver's license with reliable transportation. Home health experience is preferred.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience (preferred).
• Home Health experience (preferred).

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the physician and health care team and participate in Plan of Care.
• Provide therapeutic treatment to improve/restore strength and function.
• Review retraining programs to align with patient goals.
• Design community reintegration activities and coordinate with the interdisciplinary team.
• Document care observations, interventions and evaluations in a timely manner.
