Job summary

Responsibilities

• Assess daily living and work-related skills; create retraining programs.
• Confer with physician and care team to Plan of Care.
• Provide therapeutic treatment per physician orders to improve strength and function.
• Review retraining programs to meet patient goals.
• Coordinate with vocational team to develop plans.
• Document patient status and progress to physicians and families.
• Participate in discharge planning and care coordination.
• Design community reintegration activities and adaptive equipment recommendations.
• Train staff and family on retraining programs.
