Job summary

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physician and care team to develop/ revise the Plan of Care.
• Provide treatment to improve/restore strength, coordination and function per physician orders.
• Review/revise retraining programs to meet behavioral goals and rehab program guidelines.
• Consult with vocational team members to support vocational plans.
• Report patient status and progress to physician and care management staff.
• Participate in care coordination and discharge planning; support other health care personnel.
• Design community reintegration activities and coordinate with interdisciplinary team.
• Recommend adaptive equipment to improve home/work environments and participation.
• Train staff and family to carry out retraining program (dressing, feeding, grooming, hygiene).
• Supervise and evaluate OT Assistant and Home Health Aide performance.
• Document care observations and submit plans and discharge summaries.
