Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to provide home-based rehabilitation services, focusing on improving strength, mobility and independence through personalized therapy plans. You will work with physicians, nurses and other professionals to implement the Plan of Care and help patients regain function in their own homes.

Required duties include assessing daily living activities, documenting progress, and coordinating care with the interdisciplinary team.

Qualifications

• Current and unrestricted occupational therapy licensure.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess and treat patients in their homes to improve strength, mobility and independence.
• Collaborate with physicians and care team to develop and revise the Plan of Care.
• Document patient progress and communicate with care management and family members.
• Supervise and train staff as needed within the rehabilitation program.
