Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to deliver in-home rehabilitation services as part of a multidisciplinary care team. You will assess daily living skills, design retraining programs, and work with physicians to optimize patient function and independence.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, ROM and function.
• Review/expand retraining programs in line with individual patient goals and rehabilitation program guidelines.
• Document patient progress and communicate with the supervisor and care management staff.
