Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to deliver in-home rehabilitation, helping patients regain strength, mobility and independence. You will assess patients, develop retraining plans and collaborate with physicians and care teams to optimize home-based recovery.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient’s physician and other health care team members and participate in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, range of motion and function.
• Review/expand retraining programs consistent with rehabilitation goals and guidelines.
• Consult with vocational team members to develop plans aligned with patient needs and capabilities.
• Report patient status and progress to physicians and care management staff.
• Participate in care coordination and discharge planning activities.
