Job summary

CenterWell Home Health seeks a Home Health Occupational Therapist to deliver in-home therapy, assess function, and plan care with physicians and rehab teams. You will supervise support staff, document care, and coordinate discharge planning while advancing patient independence.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, range of motion and function.
• Review/expand retraining programs to align with patient goals and rehab program guidelines.
• Consult with vocational team members to plan needs-based rehabilitation.
• Report patient status and progress to physician and care management staff.
• Participate in care coordination and discharge planning activities; support other health care personnel.
• Design community reintegration activities to assist the client physically and psychologically; coordinate with the team.
• Recommend adaptive equipment to improve home/work environments.
• Train staff and family to carry out retraining activities and participate in team meetings.
• Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance.
