Job summary

CenterWell Home Health is seeking a dedicated Home Health Occupational Therapist to help patients regain independence in their homes. You will assess, plan and administer rehabilitation programs, coordinating with physicians and the care team to optimize mobility and function.

This role includes a $10,000 sign-on or relocation assistance, tuition reimbursement, and a company vehicle with fuel benefits. You’ll contribute to care plans, discharge planning and community reintegration efforts.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient’s physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function.
• Review/expand retraining programs in line with patients’ behavioral goals and rehab program guidelines.
• Consult with vocational team members to develop and implement vocationally oriented plans.
• Report patient status and progress to the physician, rehab staff, and family.
