CenterWell Home Health is seeking a dedicated Occupational Therapist to deliver at-home rehabilitation, focusing on improving strength, range of motion and functional independence for patients in their own homes.

You will assess living skills, coordinate with physicians and care teams, and design retraining plans tailored to each patient’s goals, while supervising support staff and documenting progress for ongoing care management.

Formación

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsabilidades

• Assess/screen patient daily living/work-related skills and develop retraining programs with measurable objectives.
• Confer with physician and care team to update the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, range of motion and function.
• Review/rework retraining programs to align with rehabilitation goals.
• Collaborate with vocational team to develop vocationally oriented plans.
• Report patient status and progress to physician and care team.
• Participate in care coordination and discharge planning activities.
• Design community reintegration activities to aid physical reconditioning and psychological adjustment.
• Recommend adaptive equipment to improve home or work environments.
• Train staff and family in retraining activities.
• Supervise OT Assistant and Home Health Aide performance.
• Document care observations and submissions to supervisor.
