Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to assess patients in their homes, develop retraining plans and work with physicians to implement the Plan of Care. You will help patients regain independence through targeted rehabilitation and functional improvement.

Responsibilities include therapy to restore strength, range of motion and coordination, documenting progress, and participating in care coordination and discharge planning.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license with auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient daily living/work-related skills and create retraining plans with measurable objectives.
• Collaborate with physician and health care team to develop/update the Plan of Care.
• Provide therapy to improve strength, coordination, ROM and function per physician orders.
• Document patient status, progress and discharge plans accurately and on time.
• Coordinate care and participate in discharge planning with the interdisciplinary team.
