Job summary

Best Home Health Providers Inc. is seeking a qualified Occupational Therapist to support patients in their homes across the Bay Area and beyond. This per diem role offers flexible scheduling and compensation per visit.

The OT will evaluate functional abilities, create personalized plans, and educate patients and families while coordinating with the care team. California licensure and NBCOT are required, with CPR and travel readiness expected.

Qualifications

• Graduate of an OT curriculum accredited by AMA and AOTA
• Registered with the NBCOT
• Active California OT license
• Minimum 2 years of clinical OT experience
• Current CPR certification (AHA)
• Valid driver’s license and reliable insured vehicle
• Willingness to travel to patient homes
• Strong communication and organizational skills

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team for seamless, patient-centered care
• Educate family members and caregivers on the treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
