Job summary

Best Home Health Providers Inc. is seeking a per diem Occupational Therapist to support patients in their homes across the Bay Area and surrounding regions.

You will assess functional abilities, create and adjust treatment plans, and guide patients in activities of daily living to promote independence. The role requires a California OT license, 2+ years of clinical OT experience, CPR certification, and a valid driver’s license with access to a insured vehicle for home visits.

Qualifications

• Minimum 2 years of clinical OT experience.
• Active California Occupational Therapy license.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle; ability to travel to patient homes.
• Strong communication and organizational skills.
• Graduate of an OT curriculum accredited by AOTA/CAAHEA.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Instruct patients on adaptive equipment, splints, and orthotic devices as needed.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Support discharge planning as patients transition out of home health care.
