Job summary

VitalCaring is hiring a Home Health Occupational Therapist to help patients regain independence, improve daily function, and safely navigate life at home. You will partner with patients, families, and interdisciplinary teams to deliver personalized, high-impact care.

You will evaluate functional status, assess home environments, develop goals, provide interventions, fabricate splints when needed, and support return-to-work through functional evaluations.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life.
• Provide therapeutic interventions to restore function, including retraining in daily living activities.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care.
• Participate in discharge planning to support successful transitions and continued progress.
