Job summary

VitalCaring, a home health and hospice provider, is hiring a Full-Time Home Health Occupational Therapist (OT) to help patients regain independence at home. You will work closely with patients, families, and the care team to deliver personalized, high-impact care in a field-based setting.

The role requires NBCOT registration, current state licensure, and at least one year of clinical experience; CPR certification and a valid driver's license are also required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT registration.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and ability to perform daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life.
• Provide therapeutic interventions to restore function, including retraining in daily living activities.
• Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate.
• Support patients returning to work through functional and work capacity evaluations.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care.
• Participate in discharge planning to support successful transitions and continued progress.
