VitalCaring Group is seeking a dedicated Occupational Therapist (OT) for home health, working in a field-based, full-time capacity. You will partner with patients and families to restore independence and enhance daily living through personalized therapy plans.

Requirements include an accredited OT degree, NBCOT certification, state OT licensure, CPR, and a valid driver’s license. Home health experience is a plus. Benefits support medical, dental, vision and retirement plans.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of 1 year of clinical experience preferred (home health a plus).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsabilités

• Evaluate patients’ functional status and daily activities.
• Assess home environments for safety and adaptations.
• Develop individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function.
• Supervise COTAs as applicable.
• Coordinate care with physicians and inter-disciplinary team.
• Document care accurately in EMR.
• Participate in discharge planning and team meetings.
