Job summary

CenterWell Home Health is seeking a licensed Occupational Therapy Assistant to support patients in their homes under the supervision of an Occupational Therapist. You will deliver therapy, assist in programs, adapt environments, and educate patients and families.

Responsibilities include documenting progress, coordinating with the care team, and contributing to discharge planning. You’ll need an OTA license and CPR certification and reliable transportation to serve the assigned area.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient’s Plan of Treatment.
• Assist in implementing vocational/education programs and activities established by an OT to restore, reinforce, and enhance task performance.
• Design/adapt equipment and living environments and fabricate devices to improve function and independence.
• Provide therapeutic treatment and instruction to patients as directed by the OT and in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function.
• Report observations to OT/Clinical supervisor and document in patient records; assist with progress reports and maintain required documentation.
• Instruct patients and family on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning and act as a resource to the care team.
• Coordinate community reintegration activities to assist physical reconditioning and psychological adjustment.
