Job summary

CenterWell Home Health is seeking a compassionate Occupational Therapy Assistant to join our home-based care team. Under supervision of a licensed OT, you will provide therapy services, adapt activities, and teach home programs to help patients regain strength and independence.

You will document progress, coordinate with the interdisciplinary team, and support discharge planning. A valid OTA license, CPR cert, and reliable transportation are required; home health experience is a plus.

Qualifications

Current and unrestricted OTA licensure.
Current CPR certification.
A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

Provide therapy services planned, delegated and supervised by the Occupational Therapist.
Assist in implementing rehab and education programs to restore function and independence.
Design/adapt equipment and living environments to improve function.
Provide therapeutic treatment to improve strength, ROM and coordination as directed.
Document progress and prepare clinical notes and progress reports.
Instruct patients and families on home programs and use of adaptive equipment.
Coordinate care and participate in discharge planning with the team.
Plan community reintegration and coordinate with the interdisciplinary team.
