CenterWell Home Health is seeking an in-home Occupational Therapist to assess, treat and rehabilitate patients in Kalamazoo, MI. You will develop retraining programs, interpret functional tests, and collaborate with physicians and the rehab team to optimize Plan of Care and patient outcomes.

The role involves 40 hours weekly with a pay range of $85,400–$117,500 per year, and requires an active OT license, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Active, unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsabilités

• Assess and screen patients' daily living and work-related skills.
• Develop therapeutic retraining programs with measurable objectives.
• Provide in-home OT treatment to improve strength, ROM and function.
• Collaborate with physicians and rehab team on the Plan of Care.
• Document care observations and progress accurately.
• Coordinate discharge planning and care transitions.
