Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist who will assess, retrain and support patients in their homes to improve function and independence. You will collaborate with physicians and the health care team to plan care, document progress and coordinate discharge planning.

The role requires OT licensure, CPR certification, and a valid driver's license with reliable transportation. Six months of Home Health experience is preferred, with a 40-hour work week and a pay range

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
• Review/expand retraining programs in line with behavioral goals and rehabilitation guidelines.
• Consult with vocational team members to develop vocationally oriented plans.
