Job summary

Enhabit Home Health & Hospice in Connecticut is seeking a licensed Occupational Therapist to support home-based patients, helping them regain daily living skills and independence.

As part of a compassionate clinical team, you will assess needs, develop treatment plans, and provide evidence-based therapies in patients' homes, with a focus on improving functional outcomes and quality of life. Experience in home health is preferred.

Qualifications

• Graduate of an approved school of Occupational Therapy and licensed as an OT in the state.
• One year of experience is preferred; exceptions may be considered with approvals.
• Medicare home health experience is preferred.

Responsibilities

• Help patients in the home improve activities of daily living and motor function.
• Develop and implement individualized treatment plans.
• Collaborate with care teams to optimize patient outcomes.
