Job summary

H.T. Prof Executive Search is seeking a licensed Occupational Therapist to provide home health OT services in Connecticut. The role involves comprehensive assessments, treatment planning, and ongoing evaluation under a medically approved Plan of Care.

The therapist guides clients in self-care, prescribes adaptive devices, and collaborates with physicians, nurses, and other staff. Requires valid OT license, CPR, and at least 1 year in home health; driver’s license and reliable transportation are

Qualifications

• Graduate of an accredited OT curriculum or eligible for the National Registration Examination.
• Current registration/license as required by state of practice.
• Has a minimum of one year experience in a home health setting.
• CPR certification.
• Demonstrates strong verbal and written communication skills and good interpersonal skills.
• Licensed driver with automobile insured and reliable transportation.

Responsibilities

• Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation under a medically approved Plan of Care.
• Documents all findings, plans, interventions, and client progress on designated forms and submits per guidelines.
• Guides and instructs clients in therapeutic self-care and activities to improve independence.
• Advises, consults, and teaches treatment procedures to clients, family members, and staff.
• Designs and/or orders adaptive devices to promote optimal functioning.
• Instructs clients and families in client-specific programs.
• Attends care conferences and participates in care coordination as needed.
• Performs other related duties and responsibilities as deemed necessary.
