Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to assess and retrain patients in their homes, collaborate with physicians, and guide care plans to improve daily living skills and functional independence.

You will contribute to a multidisciplinary team, document progress, and participate in discharge planning while supporting patients’ rehabilitation journeys and return to independence.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Preferred six months of occupational therapy experience.
• Home Health experience.

Responsibilities

• Assess and screen patients' daily living/work-related skills and develop therapeutic retraining programs.
• Confer with physicians and rehabilitation team to refine the plan of care.
• Provide therapy to improve/restore strength, coordination and function.
• Document patient progress and ensure accurate care records are maintained.
• Coordinate discharge planning and support care team with rehab goals.
