Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to provide in-home rehabilitation under the Home Health Branch Administration. You will assess daily living skills, develop retraining programs and deliver therapy to restore strength, coordination and independence in patients' homes.

You will collaborate with physicians and the care team, document care observations, and participate in care coordination and discharge planning.

Qualifications

• Active and unrestricted OT license.
• CPR certification required.
• Valid driver's license and reliable transportation.
• Home Health experience preferred.

Responsibilities

• Assess daily living skills and plan retraining programs.
• Administer tests and interpret results to guide care.
• Collaborate with physicians and care team on the plan of care.
• Provide therapeutic treatment to improve strength and range of motion.
• Document care observations and progress accurately.
• Coordinate discharge planning and community reintegration.
• Design adaptive equipment and train staff and family.
• Supervise OT Assistant and Home Health Aide performances.
