Job summary

CareRing Health seeks an experienced occupational therapist to provide home-based OT services in accordance with regulatory guidelines. You will assess clients, develop and implement plans of care, and coordinate with physicians, RNs, and supervisors to meet clinical goals.

You will educate clients and families, supervise assistants, and document outcomes in a timely, compliant manner while applying evidence-based techniques to improve function and independence in activities of daily living.

Qualifications

• Holds a current Occupational Therapy License.
• Bachelor's degree in Occupational Therapy.
• CPR certification and ability to work with minimal supervision.

Responsibilities

• Assess clients using age-appropriate norms and development principles.
• Formulate plans of care with client, supervisor, and physician approval.
• Identify unmet client needs and refer to other disciplines as needed.
• Educate client and caregiver to enhance self-care abilities.
• Deliver services cost-effectively and efficiently.
• Provide occupational therapy in the home as prescribed by a physician.
• Report client response and changes to the physician or case manager.
• Retrain function by improving strength, ROM, and coordination.
• Guide use of therapeutic activities to improve independence.
• Provide sensory or perceptual motor training when needed.
• Provide splints or devices to improve function and prevent deformities.
• Instruct clients and team on OT program and assistive devices.
• Develop maintenance plans and home environment adaptations.
• Document assessments, interventions, and outcomes per policy.
• Utilize electronic documentation and meet submission deadlines.
• Participate in case conferences and discharge planning.
• Supervise OTAs and HH Aides; document supervision and evaluation.
• Adhere to federal/state regulations and professional standards.
