Job summary

Premier Home Health Providers Inc. is seeking an Occupational Therapist to help patients regain independence at home and improve daily living activities. You will assess homes, tailor treatment plans, and guide families to support recovery in a compassionate, home-based setting.

Role is per diem with a per-visit pay rate of $110–$130. Requires CA OT license, NBCOT, CPR, travel to patient homes, and 2+ years of clinical OT experience; flexible schedule with potential to convert to full-time based

Qualifications

• Graduate of an OT program accredited by the AOTA/COCA (education documented).
• Registered with NBCOT.
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification (AHA).
• Valid driver's license and reliable vehicle for travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the skilled service.
• Support discharge planning as patients transition home from care.
• Perform other duties as required for patient services.
