Job summary

Premier Home Health Providers Inc. is seeking a per diem Occupational Therapist to help patients regain independence and improve daily living at home across California.

You will assess home environments, create personalized plans, educate families, and coordinate with the care team to ensure safe, effective recovery. Responsibilities include supervising aides, supporting discharge planning, and traveling to patient homes.

Qualifications

• Graduate OT program accredited by AMA/AOTA.
• Registered with NBCOT/NCRA (AOTA) exam.
• Active California Occupational Therapy license.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living affected by illness or injury.
• Recommend adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the skilled service.
• Support discharge planning as patients transition home.
