Best Home Health Providers, Inc. in California is seeking a per diem Occupational Therapist to support patients in their homes. You will assess functional abilities, design treatment plans, and teach daily living skills to help patients regain independence and safely return home.

The role emphasizes teamwork, compassionate care, and clear communication with physicians and caregivers. A California OT license, NBCOT registration, CPR certification, and reliable travel to patient homes are required.

Competenze

• Graduate of an occupational therapy curriculum accredited jointly by AMA and AOTA
• Registered with NBCOT (national certification) and eligible
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver license and insured vehicle; ability to travel to patient homes
• Strong communication and organizational skills

Mansioni

• Evaluate patients in their home to identify functional needs and safety concerns
• Develop personalized treatment plans and share with physicians and care team
• Teach and retrain patients in daily living activities
• Recommend adaptive equipment and orthotic devices
• Maintain accurate clinical records and report changes promptly
• Coordinate with care team for seamless, patient-centered care
• Educate family members and caregivers on treatment approach
• Supervise OT Assistants and home health aides when OT is the sole skilled service
• Support discharge planning as patients transition home
