Job summary

Premier Home Health Providers Inc. is seeking an Occupational Therapist to help patients regain independence and safely navigate daily living at home across California. The role emphasizes compassionate, patient-centered care with collaboration among the care team and family members.

The OT will evaluate home environments, develop treatment plans, and coordinate with physicians. A CA license, NBCOT registration, CPR, and a valid driver's license are required, with per diem visits and mileage

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AHA/AOTA program.
• Registered with NBCOT (National Registration Examination).
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license and ability to travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
