Job summary

CARE FIRST HOME HEALTH CARE INC is seeking a licensed Occupational Therapist to provide home health OT services under clinical supervision. The role emphasizes delivering care in the patient’s home and adhering to the service agreement.

You will evaluate function, develop treatment plans, collaborate with physicians, and supervise therapy assistants as needed. Requires CPR certification and a valid driver’s license.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA and AOTA.
• Registered with the National Registration Examination of the AOTA.
• Licensed as an occupational therapist by the appropriate state board.
• Two years of clinical experience as an occupational therapist.
• Demonstrates good verbal and written communication, and organization skills.
• Possess and maintains current CPR Certification.
• Licensed driver with automobile in good working order and insured.

Responsibilities

• Evaluates patient’s functional status and home environment for barriers to independence.
• Develops treatment program and sets goals for improved function; communicates plan of care to physicians and staff.
• Instructs and retrains patients in daily activities; may fabricate splints and use adaptive equipment.
• Trains patients in prosthetic and/or orthotic devices as needed.
• Maintains clinical records and reports changes to supervising physician.
• Maintains communication with care team to ensure coordinated care.
• Supervises certified occupational therapy assistants as required by policy and state regulations.
