Job summary

HCR Home Care seeks a licensed Occupational Therapist to provide direct OT services in patients' homes as prescribed by the attending physician. The role involves teaching activities of daily living, managing household routines, and training on prosthetic/orthotic devices and adaptive equipment.

You will educate patients and families, monitor falls risk, and document patient progress for the physician and care team.

Qualifications

• Current, valid OT license issued by NYS Department of Education.
• One year of practical experience as a licensed occupational therapist.

Responsibilities

• Instruct patients and families in daily living activities.
• Establish household management routines.
• Instruct patients and families in the use of prosthetic, orthotic and assistive devices.
• Educate patients and caregivers in the use of adaptive equipment.
• Manage falls risk through evaluation, care planning and education.
• Record and report patient responses to physician.
• Communicate with other health team personnel.
• Prepare daily progress notes for the clinical record.
• Attend case conferences and staff meetings.
• Direct or assist with home health aide service delivery.
• Other duties as assigned.
