CommonSpirit Health at Home is seeking a compassionate Occupational Therapist to provide skilled therapy to patients in their homes. You will assess physical, cognitive, and psychosocial abilities, create and implement treatment plans, and educate patients and families on home safety and daily activity strategies.

The role requires an OT license in the practicing state, CPR/BLS certification, and at least 1 year of clinical OT experience.

Qualifications

• Bachelor's degree from an accredited OT program.
• Active OT license in the state of practice.
• Current CPR/BLS certification.
• At least 1 year of OT experience in a clinical setting.
• Valid driver's license.
• Knowledge of federal, state and local laws and regulations.

Responsabilités

• Assess patients' physical, cognitive, and psychosocial abilities to determine therapy needs.
• Develop and implement personalized treatment plans that may include exercises, adaptive techniques, and assistive devices.
• Teach patients and family members about home safety and strategies to manage daily activities.
• Maintain accurate records of patient progress and treatment plans for compliance.
