Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to join our home-based care team. Reporting to the Home Health Branch Administration, you will assess patients in their homes, develop retraining plans and collaborate with physicians to support rehabilitation and independence.

Required: active OT license, CPR, valid driver's license and reliable transportation. Preferred: 6 months of OT experience and Home Health background.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience (preferred).

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs.
• Administer and interpret diagnostic and prognostic tests of function in patients’ homes or other settings.
• Confer with patient’s physician and rehab team to update the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Review/expand retraining programs to align with patient goals and rehab guidelines.
• Consult with vocational team members to design vocationally oriented plans.
• Document patient status, interventions and progress to the supervisor and care management staff.
