CenterWell Home Health is seeking a licensed Occupational Therapist to join our patient-centered care team. You will assess daily living skills, design retraining programs, and provide therapeutic interventions in patients' homes under physician orders, aiming to improve strength, mobility and independence.

You will collaborate with physicians and a multidisciplinary team to update care plans, document progress, and support discharge planning.

Competenze

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience.

Mansioni

• Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret functional tests in patients' homes to assist physicians.
• Confer with physicians and care team to update the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, range of motion and function.
• Review retraining programs in line with patient goals and rehab program guidelines.
• Consult with vocational team to develop rehab plans aligned with patient needs.
• Report patient status and progress to physicians and care team, patients and families.
• Participate in care coordination and discharge planning and assist other clinicians.
• Design community reintegration activities and coordinate with the interdisciplinary team.
• Recommend/adapt adaptive equipment to improve home or work environments.
• Train staff and family in retraining program activities including dressing, feeding, grooming, and hygiene.
• Supervise and evaluate OT Assistant and Home Health Aide performance.
• Document care observations and evaluations thoroughly and promptly.
• Ensure orders from physicians are accurately implemented.
