Job summary

Always Best Care of Greater Cleveland is seeking an Occupational Therapist to deliver home-based therapy across Ohio. You will evaluate patients, set goals, and provide therapy in real-life home environments, collaborating with families and care teams.

Responsibilities include supervising OTAs, educating patients and caregivers, documenting outcomes, and helping patients regain independence in daily activities. CPR certification and Ohio licensure required.

Qualifications

• Licensed OT in Ohio.
• Graduate of an accredited OT program (MS/OTD or BS equivalent).
• Current CPR certification.
• Excellent communication and teamwork skills.
• Willingness to travel for home visits.

Responsibilities

• Evaluate patients and create personalized home-based treatment plans.
• Set meaningful short- and long-term therapy goals.
• Provide therapy in real-life home environments.
• Collaborate with patients, families, caregivers, and care team.
• Recommend or create adaptive equipment when needed.
• Track progress and adjust plans as patients improve.
• Complete accurate and timely documentation.
• Educate patients, families, caregivers, and staff.
• Supervise and support Occupational Therapy Assistants (OTAs).
• Help patients become more independent and confident in daily life.
