Job summary

Enhabit Home Health & Hospice is seeking a licensed Occupational Therapist to deliver home health OT services, helping patients regain daily living skills and independence. You will work under a physician-ordered plan of care and coordinate with the care team to support in-home recovery.

The role favors candidates with Medicare home health experience and at least one year of prior OT practice. Licensed OT in the state of practice is required, with opportunities to advance within a collaborative

Qualifications

Must be a graduate of an approved school of Occupational Therapy and licensed as an Occupational Therapist in the state of practice.
Minimum of one-year experience preferred, but may be considered with VP/Regional Director approval based on prior experience.
Medicare home health experience is preferred.

Responsibilities

Help patients with debilitating conditions improve daily living activities and basic motor functions in the home.
Provide skilled OT services per physician orders and plan of care under supervision of Branch Director.
Educate patients, families, and health-care team members to support ongoing care in the home.
