Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to help patients regain independence in their homes. This full-time, field-based role partners with patients, families, and the care team to deliver personalized, high-impact therapy.

You will evaluate functional status, develop goals, fabricate splints as needed, and participate in discharge planning to ensure smooth transitions and continued progress.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification.
• Current state OT license.
• Minimum of one year of clinical experience preferred (home health a plus).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status and daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals.
• Provide therapeutic interventions to restore function, including retraining in daily living activities.
• Fabricate splints and train patients in the use of adaptive devices.
• Support patients returning to work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on treatment plans and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care.
• Participate in discharge planning to support successful transitions and continued progress.
• Supervise COTAs in accordance with state regulations and company policy.
• Provide oversight of home health aide care as required when therapy is the sole skilled service.
