Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to evaluate patients, design personalized treatment plans, and help patients regain independence in daily activities at home. You will collaborate with families and the care team to deliver patient-centered care and ensure safe transitions.

The role requires an accredited OT degree, NBCOT registration, state licensure, and CPR certification. A driver’s license and reliable transportation are also needed for field-based work.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT registration and state OT licensure.
• Minimum of one year clinical experience preferred, home health a plus.
• CPR certification and valid driver's license.

Responsibilities

• Evaluate patients’ functional status and daily activity performance.
• Develop individualized treatment plans to improve independence and function.
• Provide therapeutic interventions and retraining for daily living activities.
• Fabricate splints and train use of adaptive devices as appropriate.
• Coordinate care with physicians and interdisciplinary teams.
• Participate in discharge planning and care conferences.
• Document care in the EMR and educate patients and families.
