Job summary

Responsibilities include comprehensive physiologic and psychosocial assessments, planning and implementing care, safe medication administration, and thorough discharge planning with families and care coordinators.

Qualifications

• Graduate from an accredited School of Nursing.
• Current license to practice as a Registered Nurse in New York State.
• BLS certification required, with specialized certifications as needed.

Responsibilities

• Assess the patient’s physiologic health status.
• Assess patient’s psychosocial health status.
• Identify patient outcomes and develop care plans.
• Formulate and prioritize the plan of care.
• Implement the plan of care and administer treatments.
• Evaluate care and document outcomes.
• Collaborate with physicians and care team in treatment planning and discharge.
• Perform skin care and wound management.
• Administer medications safely and educate patients on drug interactions.
• Engage in patient and family education and discharge planning.
