Job summary

Northwell in New York is seeking a dedicated registered nurse to deliver age-specific care using the nursing process. You will collaborate with physicians and the health team to coordinate procedures and treatments, applying leadership to guide the care unit.

The role requires interviewing patients, assessing multiple health domains, documenting care plans, and educating patients. You will participate in discharge planning and referrals to ensure optimal outcomes.

Qualifications

• Must have completed nursing education and be eligible for NY State licensure.
• Willingness to pursue BSN within the specified timeframe.
• Ability to document and communicate patient status and care plans.

Responsibilities

• Performs nursing process: assessment, diagnosis, planning, implementation, and evaluation.
• Collaborates with physicians and health team to coordinate care.
• Interviews patients, explains policies, and reviews charts accurately.
• Assesses GI, CV, respiratory, renal, neuro status and pain levels; adjusts care plan.
• Documents health status, care plans, and patient outcomes clearly.
• Provides discharge planning, referrals, and education for patients and families.
