Job summary

Granite Peak Home Health is seeking a registered nurse licensed in Montana to deliver high-quality home health services. Responsibilities include initial evaluations, creating and updating plans of care, and coordinating with the care team to meet patient needs.

The role requires independence, strong clinical judgment, and familiarity with HIPAA and local healthcare regulations. A valid driver's license and WellSky EHR proficiency are preferred, with on-call duties as needed.

Qualifications

• Must be licensed in Montana.
• Two years nursing experience with 1 year Home Health preferred.
• BLS/CPR certification required.
• Familiarity with HIPAA and patient privacy requirements.
• Knowledge of local healthcare market and outreach strategies.
• Proficient with WellSky EHR and Microsoft Office.

Responsibilities

• Performs initial evaluation and regularly reevaluates patient nursing needs.
• Initiates and revises plan of care.
• Provides services requiring specialized nursing skills.
• Prepares clinical and progress notes promptly per policy.
• Coordinates services and informs physicians of changes.
• Counsels patient and family on nursing needs.
• Gives total patient care and participates in peer reviews.
