Job summary

Greater Springfield Senior Services, Inc. is seeking a full-time registered nurse to perform nursing duties under the Clinical Assessment and Eligibility (CAE) and State Home Care programs.

This on-site role requires coordinating care screenings to determine medical eligibility and handling duties under the Coordination of Care and Waiver programs. Responsibilities include completing initial and annual assessments, developing care plans, maintaining paper and electronic files, advising on

Qualifications

• Graduate from a NLN-accredited nursing school.
• Bachelor of Science in Nursing or an Associate of Science in Nursing with at least one year of clinical/community nursing experience.
• Or a Diploma RN with two additional years of community-based nursing experience.

Responsibilities

• Complete initial assessments and annual reassessment visits for personal care and home health aide clients.
• Develop appropriate care plans for personal care clients.
• Prepare and maintain nursing paper and electronic files.
• Provide consultation on Protective Services, Elder at Risk and Respite Care cases.
• Act as a liaison with agency providers.
• Serve as a resource on medical matters for all staff.
• Act as backup to nurses in other medical programs.
• Be an active member of the interdisciplinary team providing nursing consultation to case management staff.
