Job summary

Enhabit Home Health & Hospice is seeking a qualified Nursing Coordinator to oversee the coordination of home health services, ensuring care aligns with standards and regulatory requirements. The role includes reviewing start-of-care documentation, supporting care plans, and coordinating with physicians, referral sources, patients, and caregivers to maintain continuity of care.

Applicants should have a nursing degree, a valid license, CPR certification, and at least two years of recent nursing

Qualifications

• Graduate of an approved school of professional nursing.
• Must hold and maintain a valid clinical nursing license per state requirements.
• Minimum two years of nursing experience within the last 36 months.
• Minimum one year of experience in home health.
• Knowledge of federal, state and local laws governing home care.
• CPR certification required.
• Valid state driver's license and automobile insurance.
• Basic technology skills including mobile devices.
• Bilingual ability preferred; English/Spanish.
• Office software proficiency (MS Word/Excel).

Responsibilities

• Coordinate home health services for patients according to standards of practice.
• Review and approve patient information submitted by licensed professionals at start of care, recertification or evaluation visits.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and coordinate with field staff to maintain care continuity.
