Job summary

Cure Home Health LLC is seeking a dedicated registered nurse to provide comprehensive in-home care. You will conduct holistic health and psychosocial evaluations, monitor patient conditions, promote preventive practices, coordinate services, and train families in the home setting.

Responsibilities

• Coordinate total patient care by conducting comprehensive health and psychosocial evaluation
• Monitor the patient's condition and teach and train activities
• Evaluate effectiveness of nursing service to patient and family
• Perform admission, transfer, re-certification, resumption of care, and discharge paperwork
• Consult with attending physician concerning alterations of Patient Care Plans
• Coordinate patient services and submit weekly clinical and progress notes
• Participate in case conferences and discuss patient issues with supervisor
• Involve Home Attendant, Physical Therapist, Speech Therapist, Occupational Therapist, Medical Social Worker as needed
• Obtain orders for paraprofessional service and submit referrals
• Participate in discharge planning and continuity of care with other agencies
• Maintain knowledge of current drug therapy and adhere to requirements
