Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed nursing experience in California.
• Current licensure in state and CPR certification.
• Bachelor’s degree, with 1 year of home health care experience preferred.
• No management experience required; supervising home health aides.
• Excellent observation, verbal and written communication skills.
• Ability to lift and transfer patients; physical stamina required.
• Valid California Driver’s License with vehicle in good working order.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Initiates preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient per plan of care.
• Identifies discharge planning needs and implements prior to discharge.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Prepares clinical notes and updates the primary physician at least every 60 days.
• Communicates with physician regarding patient needs and reports changes in condition.
• Coordinates the care plan with community health-related persons.
• Participates in on-call duties as defined by policy.
• Ensures arrangements for equipment and services are available.
• Instructs, supervises, and evaluates home health aides.
• Adheres to attendance and reliability guidelines.
