Job summary

Home & Health Care Management, Inc. seeks a Registered Nurse to perform intake, assessments, and care coordination in home health settings across Sacramento County and surrounding areas.

You will work with a multidisciplinary team to enroll clients into HCBA Waiver, CCT, and ALW programs, and ensure timely referrals when gaps in care appear. Responsibilities include conducting initial in-home assessments, coordinating with HCBA RN and social work staff, and maintaining documentation through

Qualifications

• Current California RN license.
• Valid California Driver’s License.
• Own vehicle with proof of current auto insurance.
• Proficiency with software programs and MS Office.
• Ability to work independently and with others at all levels.

Responsibilities

• Review medical records and plans of treatment to ensure care meets client needs.
• Communicate with providers, patients, and care teams about client needs.
• Collaborate with HCBA team and case management staff on enrollment and care coordination.
• Track quality indicators and reports to improve patient outcomes.
• Conduct monthly visits with participants via phone, Teams, or in person.
• Perform initial assessments for HCBA waiver enrollment.
