Pride Health is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing in patients' homes across Grady, Decatur, Mitchell, Seminole, and Early Counties in Georgia. The role offers Monday–Friday, 8:00 AM–4:30 PM, with a pay rate of $28.00–$30.00 per hour and mileage reimbursement between visits.

Pride Health is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout Grady, Decatur, Mitchell, Seminole, and Early Counties. This position plays a key role in delivering high-quality patient care, coordinating with interdisciplinary healthcare teams, documenting clinical services, and supporting positive patient outcomes in the home health setting.

• Location: Grady, Decatur, Mitchell, Seminole, and Early Counties, GA
• Patient Care Setting: Home Health
• Schedule: Monday-Friday, 8:00 AM - 4:30 PM
• Pay Rate: $28.00-$30.00/hour
• Mileage Reimbursement: Yes, reimbursed between patient visits (first patient to last patient)

Key Responsibilities

• Provide skilled nursing services in accordance with the plan of care, physician orders, agency policies, and state nursing practice regulations.
• Deliver patient care while following all infection control and safety standards.
• Document patient visits thoroughly and accurately within the electronic medical record and electronic visit verification systems.
• Monitor patient conditions and promptly communicate changes, concerns, and care needs to physicians and other members of the healthcare team.
• Participate in interdisciplinary care coordination activities, including case conferences and collaboration with caregivers, providers, and community resources.
• Educate patients and caregivers regarding treatments, medications, disease management, and care plans.
• Provide compassionate care to diverse patient populations while maintaining patient dignity and independence.
• Ensure compliance with all regulatory, agency, and payer documentation requirements.

Required Qualifications

• Current unrestricted Licensed Practical Nurse (LPN) license.
• Previous home health experience.
• Experience using Homecare Homebase (HCHB).
• Strong clinical assessment and documentation skills.
• Ability to travel throughout the assigned coverage area.
• Excellent communication and organizational skills.

Preferred Qualifications

• Advanced proficiency with Homecare Homebase (HCHB).
• Experience managing a home health patient caseload.
• Ability to work independently with minimal supervision.
• Strong critical thinking and time management skills.
• Experience coordinating care across multidisciplinary teams.
